Your Initial Visit

You initial consultation at GHFM Clinic is our chance to really get to know you and your health care needs. Our intention is for every patient to leave their consultation empowered to take meaningful steps toward their best health that is applicable to them personally.

It is essential you complete your new patient paperwork at least 7 business days prior to your initial appointment. This will help you make the most of your face-to-face time with Dr. E

At your first appointment we will:

  • Do a deep dive into your health history

  • Perform pertinent physical exam

  • Provide nutritional guidance

  • Create an individualized treatment plan focused on your health goals and condition by using multiple systems of medicine. The treatment plans are customized and include nutrition recommendations, basic supplements, medications if needed, mind body strategies and exercise suggestions.

  • Provide recommendations regarding any necessary laboratory testing.

  • Food allergy testing / evaluation for Celiac disease

  • Stool testing and microbiome analysis (using DNA PCR technology)

  • Evaluation for leaky gut and SIBO

  • Genetic testing

  • Complete Hormone testing (including cortisol, DHEA, estrogen, progesterone, testosterone, thyroid & melatonin)

  • Micronutrient testing

  • Organic acids profile (including analysis of mitochondrial energy production, detoxification, methylation, neurotransmitter production, carbohydrate & fat metabolism, oxidative stress evaluation)

Your Initial Follow-Up Visit

  • The second visit is generally scheduled 4-6 weeks later, since specialty labs have a longer turn around time.

  • Prior to this visit, Dr. E thoroughly reviews and analyses your case.

  • During the visit Dr. E will discuss the review of findings. This includes what may be causing your health problems and what supplementation (vitamins, minerals, and herbs), diet, and life style may be needed, as well as any other medications that may be appropriate for your care.

Subsequent Follow-Up Visits

Subsequent follow up visits are usually scheduled at approximately 6-8 week intervals. Dr. E continues to monitor your progress and makes adjustments in your program to address all key areas of dysfunction, until symptoms are managed and you appear on a clear path to wellness.

Long-Term Wellness

After most of your symptoms have resolved, you have the option to continue follow up visits (1 -2 time per year) for health optimization and continued wellness.

Health Maintenance

HEALTHY AGING: At GHFM clinic, we believe in disease prevention. Imbalances in cellular function occur long before disease manifestation. If you currently have no major complaints and are simply interested in optimizing your health and preventing disease in the future, we offer yearly in depth analysis of your micro nutrient status and assess any abnormalities in you cellular function. We then design targeted personalized nutrition and supplement programs to bring your body back in balance and recharge your metabolism.

HEALTHY WEIGHT: Excessive weight resistant to conventional diet and exercise programs may have many underlying reasons, like hormonal and metabolic imbalances, changes in your gut microbiome, leaky gut, chronic inflammation, nutritional insufficiency, and environmental toxins. There is no quick pill that fixes all the underlying causes.

At GHFM Clinic, we do advanced functional testing to determine the precise reasons for excessive weight and based on your results, we develop personalized nutrition and supplement programs that aim to balance your body’s chemistry and bring you back to your ideal weight.

Our Fee Schedule

We strive for your experience at The Green Health Functional Medicine Clinic to be an excellent one. In order to achieve that goal, we want you to be fully informed of our policies. In order to make the best use of your time with the doctor, we encourage you to write down any questions, concerns, and other discussion points for your visit and to complete the questionnaires in the Patient Portal at least 24 hours prior to your appointment.

Payments & Fees

We require payment on the day of service. We accept Visa, MasterCard, Amex, and Cash payments.

  • New Patient Appointment: $350-$500 (60 min)

  • Extended Follow-up Appointment (Over 2 years since the last visit): $350 (60 min)

  • Initial Follow-up Appointments: $350 (60 min)

  • Follow-up Appointments: $250 (30 min)

  • Phone Appointment/Follow up Questionnaire Appointment: $100 (30 min) | $75 (15 min)

Phone/Email Consults

If you need an immediate response or medical attention, email is not the right communication tool. We suggest that you schedule an office visit or phone appointment with our physician or go to the Emergency Room.

  • E-mail or “Portal” communication that requires more than 5 minutes of attention from your physician or medical staff will be subject to a fee of $125 for every 15 minutes.

  • If requests/questions require medical decisions the email will be sent back to the patient with a request of an office or phone visit with the Doctor.

  • All emails will be responded to within 48hrs.

Cancellation Fee

We require a 7-day notice if you need to cancel office visits or phone visits. If no notice is given there will be a$ 100.00 cancellation fee.

Primary Care

All patients are required to have a primary care physician for their routine medical and medication needs. The Green Health Functional Medicine Clinic is a consultation service only and does not provide primary care service or emergency service.

At any time an urgent health issue arises, seek an urgent care clinic or Emergency Room care.

Medical Records

Your privacy is of utmost importance and protected by law.

Your medical records are subject to HIPAA policies. We need direct written consent in order to release records regarding your care with us to anyone other than directly to you and have forms available in your patient portal and in the office for your convenience.

Discussion of your treatment with others requires your written permission even if it is a family member or spouse.

Administration Fees

At times, we are asked to fill out forms for work, insurance companies, or other physicians, etc. In order to comply with these requests in a timely manner, we charge a minimum of $50 case analysis fee. That fee can vary depending on the size of the chart for records or documentation to create and fill out.


We are an insurance-free practice and a low volume office, offering our patients comprehensive care, expecting payment at the time of service. Though we are not on any insurance plans and are considered out-of-network, we will provide the necessary information including a medically coded receipt for you to submit to your insurance company. After payment is made to our office, the bills can be submitted to your insurance company and will be reimbursed to you dependent upon each individual insurance plan. If desired, please check with your insurance company prior to your visit. We are not Medicare providers. However, we may be able to order any tests needed for Medicare patients and these will be covered as usual under Medicare guidelines.

  • What happens after I pay for my visit? How do I get reimbursed?
  • We are an insurance-free practice, and we are not contracted with Medicare. After payment is made at the time of service to our office, you will receive a statement listing all services properly coded by the physician that may be submitted to insurance plans directly by you to the address typically found on your insurance card. You will be reimbursed dependent upon your individual insurance plan. You may choose to contact your insurance company prior to your visit for information on your coverage as well as options to use a “wellness benefit” that may provide some reimbursement. You have a contract with your insurance company; we do not. You will be provided with documentation at the time of service; submit a copy and keep the original. Additionally, we are not providers for any HMO insurance or Medicare. The Green Health Functional Medicine Clinic are opted out of Medicare and all patients are required to pay at the time of service and may not be eligible for reimbursements from secondary coverage. Some services may be covered by Medicare MAY or MAY NOT provide reimbursement or payment for certain lab tests.
  • Why we require payment up-front and charge for missed appointments?
  • The Green Health Functional Medicine is a low volume office. We do not double book patients – a practice which is common in most physicians’ offices. Your booked appointment is dedicated to your time and we run on time, to avoid the long wait common in most physician’s offices. Your visit is reserved in advance for you so your personal clinician can focus on your comprehensive care. Insurance companies expect Clinicians to double book an average of 6 patients per hour which would not allow us to provide our patients with the comprehensive integrative care that our center is known for. Since we do not overbook and schedule one patient at a time, (initial visits vary from 1 to 1 1/2 hours), your missed appointment represents a loss for our physicians and to the center, therefore we require a 7 day notice of cancellation for all appointments.

Patients partner with Dr. E to resolve specific conditions or symptoms to optimize their health. He believes in personalized medicine and works with her patients to meet their health goals.

Current Billing Rate is $350 to 500 per/hr depending on depth of medical records for review.

Initial Consult: 60 mins

An in-depth medical, diet, and lifestyle history intake.

A personalized healing plan which may include:

  • Laboratory and advanced functional medicine tests

  • Recommended nutritional changes

  • Lifestyle modifications

  • Medical-grade supplements

  • Stress management

  • And other integrative functional medicine strategies

The intention is for every patient to leave their consultation empowered to take meaningful steps toward their best health. A comprehensive food plan, recipes, and numerous informative handouts are provided to guide each patient in their next steps toward whole health.

  • Video consultation, phone or other forms of communication may be utilized depending on your preferences, location, and needs.

Please note: Depending on the severity of your clinical condition and complexity of your case, extra time may be needed. Additional billing is applied at $7.5 per/min.

Initial Follow-Up Visit: 60 mins @ $300/hr

Review, interpretation and an in-depth analysis of your lab work.

  • Personalized plan of care based upon your lab findings.

  • A more customized plan of care that addresses your particular needs. This may include adjustments to your initial dietary plan and a more tailor-made supplement program.

  • Video consultation, phone or other forms of communication may be utilized depending on your preferences, location, and needs.

Please note: Depending on the severity of your clinical condition and complexity of your case, extra time may be needed. Additional billing is applied at $7.5 per/min.

Follow-Up Visit: 30 mins @ $250/hr

These visits with Dr. E focus on review of any follow up lab results, your physical response to dietary, exercise, and/or lifestyle modifications.

  • Follow-Up visits are scheduled every 8-10 weeks, till most of your symptoms have resolved.

  • Enhancements or adjustments made to your healing plan (based on lab work, progress, symptoms, etc.).

  • Video consultation, phone or other forms of communication may be utilized depending on your preferences, location, and needs.

Please note: Depending on the severity of your clinical condition and complexity of your case, extra time may be needed. Additional billing is applied at $7.50 per/min.

Lab Testing

  • Functional lab testing helps evaluate health on a cellular level. This testing is different from traditional testing because it helps evaluate the root cause(s) of your health challenges instead of just diagnosing you. This testing is done through urine, stool, saliva, and/or blood.

  • The lab recommendations will depend on the complexity of your case and Dr. E will discuss them with you at time of your Initial Consult.

  • Screening labs to be done through Lab Corp or Health Lab which is billed through insurance prior to the appointment.

  • The cost of Functional Labs is not included in the Essential or the Comprehensive Plan.

Lab Testing Refund Policy

  • Labs may be returned within 30 days of purchase. Patients will need to return the test kits back to the office before any refund is issued.

Supplement Return Policy

  • Unopened/sealed supplement bottles purchased in the office will be eligible for a full refund within 30 days of purchase. No refund/exchange will be given after 30 days of purchase.

  • The return policies established by Fullscript will apply, on any purchases made on their websites.

Insurance Details

  • GHFM Clinic is a fee for service practice, and we do not accept any medical insurance.

  • We will not be able to follow up on billing inquiries made by insurance companies on your behalf. It is your responsibility to work with your provider to answer any additional questions they may have.

Cancellation Policy

  • You may cancel your plan for up to 5 business days without any charge.

  • Since a lot of time and effort is put in initially to set up your plan, any cancellation after 5-days from starting the program will require the proration of the case review and administrative charges.

  • Consult appointments for your Plans may be moved 5 days earlier/later than your scheduled date

Patient Portal Messaging Policy

  • Messages sent through the Patient Medical Portal for simple straightforward questions regarding your existing treatment plan, requiring simple ‘Yes’ and ‘No’ answers will be addressed.

  • The weekly Group Q&A sessions with Dr. E will be used for more detailed questions.

Other Notes

Consultation are time recorded for billing purposes at $350-500 per hour. Depending on the severity of your clinical condition and complexity of your case, if extra time is needed beyond your scheduled appointment time – additional billing will be applied at $7.5 per/min.

Messages sent through the patient medical portal for simple straightforward questions regarding your existing treatment plan, will be answered. However, messages that require changes to your existing plan, new prescriptions or complex decision making – you will be asked to schedule a follow up appointment. We feel that important details about your health may be missed through back-and-forth messaging.
